The question underneath the question is usually this: someone put a camera in your line, showed you something ugly, and told you the only fix is tearing out everything. Whether that's true comes down to what the camera actually shows — how bad the defect is, and how far it runs.

A cast iron failure isn't one thing. The North American standard for grading a sewer on camera scores a pipe on a 1-to-5 scale, and it weighs two separate things: the severity of the defects and the extent of the defects. A deep defect in one spot is a different pipe than a shallow defect running the whole length. That distinction is the entire difference between a spot repair and a replacement.

What a spot repair actually is

A spot repair is real plumbing, not a bandage. Here's what it is, step by step.

It starts at the monitor, not the saw. The camera locates the failure and measures how long it runs — that's the number that decides everything. Guessing at it is how a small failure becomes a whole-floor invoice.

The cut-out. Old cast iron that's failing is brittle and graphitized — the metal has corroded down to something closer to soft graphite than solid iron. You don't take a reciprocating saw to it; the vibration shatters the pipe further down the line and turns a small repair into a big one. A chain snap cutter wraps hardened cutting wheels around the pipe and ratchets down, snapping it clean and square in one motion without cracking the pipe you're keeping.

The splice. New Schedule 40 PVC goes in where the dead iron came out. PVC joints aren't glued — they're solvent-welded, meaning the primer and cement chemically soften the plastic so the two pieces fuse into one continuous piece. The purple primer is the tell: the code that a licensed plumber has to build to says a purple primer conforming to the ASTM F656 standard “shall be applied” before the cement. It's dyed purple on purpose, so an inspector can see the joint was prepped and not dry-fit. Skip it to save fifteen minutes and the weld is weak from the day it's buried.

The transition joint — where the repair lives or dies. You can't chemically weld PVC to iron; they're different materials. The code requires a joint between two different pipe materials to be a mechanical compression or sealing coupling. For cast iron, it goes further: the coupling has to be the shielded type — an elastomeric sleeve wrapped in a metallic shield. That shield, stainless steel under the ASTM C1277 standard, is what holds the two pipes in rigid alignment. A plain rubber sleeve with no shield deforms under the weight of soil and slab, the PVC drifts downward, and you've built yourself a fresh belly that catches waste.

Here's the tactic to watch for. A plumber runs a camera, points it at a rust flake or drags the lens through a clearable clog until the picture goes muddy, and announces the whole system has collapsed. The footage is either from the worst few inches of your line or, in the uglier version, pre-recorded from someone else's. Then comes the pitch: the only fix is a full under-slab repipe.
