That one distinction decides whether a tank is a real safety requirement or a line item padding an invoice. Here's how to tell which side of it you're on, and what the code actually says.

If you can find a one-way device on your main line, you're closed and a tank is genuinely required. If nothing on the line stops water from flowing back to the street, the IPC doesn't require one — and anyone telling you otherwise is selling, not diagnosing.
The mechanical reason the loop matters
Water expands when you heat it. Your water heater warms 40 to 50 gallons, and that heated volume takes up more room than it did cold.
On an open system, that extra volume has an exit — it pushes harmlessly back into the city main. Nothing builds up.
Put a one-way device in the line — a PRV, a check valve, or a backflow preventer — and you've sealed the main off. Now the expanding water has nowhere to go. Pressure climbs, often past 150 PSI, well beyond what your temperature-and-pressure (T&P) relief valve is built to sit at. The valve starts weeping just to bleed the pressure off. That drip is the symptom; the sealed loop is the cause.
A thermal expansion tank is the cushion that absorbs it. Inside a steel shell is a heavy-duty butyl rubber diaphragm splitting the tank into a water chamber and a dry air chamber, factory pre-charged to 40 PSI, with a Schrader valve on top — the same valve stem as a bike tire. When the heater cycles and pressure rises, water pushes the diaphragm against the air cushion instead of against your T&P valve.
The one step that makes or breaks the whole installation is the pre-charge. That factory 40 PSI almost never matches a real house. We put a liquid-filled gauge on your exterior hose bibb, read your actual static pressure — say 65 or 72 PSI — then charge the tank's air side through the Schrader valve with a hand pump or compressor until it matches, before water ever touches it. Skip that and the diaphragm collapses under municipal pressure, and the tank water-logs within months. At that point you paid for a tank that does nothing.
Then it gets hard-piped to a rigid brass tee on the cold inlet and physically supported. A water-logged tank holds roughly two gallons and can top 20 pounds. Hang that off a flex hose or a sagging PEX line and you've put shear stress on a joint that was never meant to carry it.

The scam: "required by code" as a weapon
The phrase "required by code" gets weaponized two opposite ways, and both are worth recognizing.
Weaponized as an upsell. Even on an open system where the IPC doesn't require a tank — or when the real fix is one small part — a predatory tech writes "mandatory code upgrade" on the invoice and quotes a "code-compliant system replacement with a new expansion tank" at $2,800 to $3,500. Then comes the offer to finance it on the spot through a GreenSky deferred-interest plan. The tank is a small brass-and-steel part. The dollar figure is a script, not a material cost. This is textbook System Replacement Fraud stapled to the GreenSky financing trap.
Weaponized by skipping it. The low-bid installer does the reverse. They set a heater on a closed system and leave the required expansion tank off — sometimes the T&P discharge piping too — to save a few dollars in parts. Now your water heater is a sealed, pressurized vessel with no cushion and a relief valve doing a job it was never meant to do alone. When a tank like that finally fails, it doesn't drip. It releases, and the water damage runs $50,000 to $150,000. That's the "Skipping TPR / Expansion Tanks" tactic, and it's the one that ends up on an insurance claim.
