Does a Tankless Water Heater Need Annual Descaling? Yes — Because Scale Insulates the Heat Exchanger Until It Overheats
Yes. A condensing tankless water heater needs a descaling flush at least once a year, and the manufacturers put it in writing. Heated hard water leaves calcium-carbonate scale inside the heat exchanger. That scale insulates the metal, makes the burner run hotter, chokes the flow, and eventually cracks the exchanger.

What scale actually does to a heat exchanger
A tankless heater doesn't store hot water. It flash-heats it on demand — cold water runs across a stainless-steel heat exchanger while a gas burner fires against the other side. The exchanger wall is the one piece of metal standing between the flame and the water, and it's the part that takes the abuse.
Now run hard water through it. When hard water is heated, the U.S. Geological Survey notes, solid calcium carbonate deposits form, and that scale “can reduce the life of equipment, raise the costs of heating the water, lower the efficiency of electric water heaters, and clog pipes.” Inside a tankless exchanger, those minerals drop out of the water at the hottest spot — the exchanger wall — and harden onto it.
Here's the part that costs you the unit: scale is an insulator. The Department of Energy pegged its thermal conductivity at an order of magnitude below bare steel, and found that “even thin layers of scale serve as an effective insulator” that retards heat transfer, leading to overheating of the metal, tube failures, and lost efficiency. That measurement came out of firetube boilers, but the physics doesn't care about the appliance — it's the same story anywhere a flame heats metal that heats water.
So picture the burner trying to push heat through a mineral blanket. The water side can't pull heat away fast enough anymore, the metal behind the scale runs hotter than it was ever meant to, and you get localized boiling and thermal stress on the exchanger wall. Run it that way over repeated heating cycles and the exchanger stress-fractures. That's the failure that ends the unit — not a worn-out sensor, but a wall of metal cooked from behind a layer of scale until it cracks.
Scale doesn't stop at the exchanger, either. The flow sensor — the little magnetic turbine that tells the unit water is moving so it should fire — sits right in that same mineral-heavy stream. Calcium can seize that turbine so it stops spinning, and with no signal that water is moving, the unit won't light at all — you open a hot tap and get nothing but cold.

Hardness gets measured in milligrams of calcium carbonate per liter. The USGS line is: up to 60 is soft, 61 to 120 moderately hard, 121 to 180 hard, and anything past 180 is “very hard.” The harder the water, the faster scale plates onto the exchanger wall.

What skipping the flush looks like over time
Scale is a slow tax. None of this shows up on day one — by the time the unit throws a code, the minerals have already been working on the metal for a while.

The scam: a “proprietary chemical flush” that’s really just vinegar
The flush itself is honest work. Where it gets bent is what the flush gets called on the invoice.
The job on the inside is white vinegar or an approved descaling solution, pumped through the heat exchanger for about an hour. That's the whole procedure. But “circulated vinegar” doesn't justify a big number, so it gets a new name — a “proprietary chemical flush.” Relabeled that way, the line item runs up to $800 for the same acid, the same loop, and the same hour of pump time.
The other version of the same con is quieter: skip the flush entirely. A shop that has already decided your next call is a replacement has no reason to dissolve the scale keeping that heater alive. Let the exchanger cook behind its mineral blanket, wait for the crack, and sell the new unit. The skipped maintenance is the sales strategy.
Some shops also fold the descale into a “membership” or “maintenance plan.” Whatever it's called, know the one number that actually matters: the exchanger needs exactly one flush a year, no more and no less, and no plan changes that.

We do it the boring way: flush the unit on schedule, tell you the date, hand back a heater that works, and leave. No “proprietary” label, no markup on a jug of vinegar. What a descale actually involves
There's no mystery to it, which is why the “proprietary” label is such a tell. The manufacturer procedure is public, and it goes like this:
Isolate the unit at its hot and cold service valves. These are the two purpose-built shutoffs on the water connections. If your unit has them, the flush is straightforward. If it doesn't, that's the first job.
Build the loop. A small submersible pump sits in a 5-gallon bucket, feeds descaling solution into the cold service port, and the hot service port drains back into the same bucket.
Circulate white vinegar or an approved descaling solution for at least 45 minutes. That's the acid dissolving the calcium off the exchanger walls.
Flush with fresh water for at least 10 minutes. This clears the acid and the loosened mineral out of the exchanger channels so nothing corrosive gets left behind.
Clean the rest while it drains. Rinse the cold-inlet filter screen, brush out the air-intake filter, and clear the condensate trap. Reseat the condensate trap O-ring correctly — a bad seal there leaks combustion gas into the house, not water.
When to do it, and what your warranty says
Once a year. Navien recommends descaling at least annually, and that interval is baked into how these units are warranted. Manufacturers don't cover scale damage — Rheem's tankless warranty, for one, excludes heat-exchanger failures caused by “lime, mineral build-up, or scale.” So the yearly flush isn't only about efficiency. It's the line between a covered repair and a replacement you pay for out of pocket because the paperwork says the scale was your job to remove.
The number that should stop you

White vinegar works. The manufacturer procedure allows white vinegar or an approved descaling solution, circulated for at least 45 minutes and then chased with a fresh-water flush. The scale doesn't know whether the acid came in a branded jug. When someone insists only a “proprietary” chemical will do, that's usually a markup talking, not chemistry.
The acid alone has to circulate at least 45 minutes to dissolve the scale, then at least 10 minutes of fresh-water flushing to clear it — so the flush is close to an hour before you add setup, teardown, and cleaning the inlet screen, air filter, and condensate trap. Anyone promising a five-minute “flush” isn't running solution through the exchanger.
Not until valves are added. The flush loop connects to the hot and cold service valves; with no valves there's no way to isolate the unit and pump solution through it. Adding a service-valve kit is a small job, and after that every future flush is routine.
A softener pulls calcium out before it reaches the heater, so the exchanger scales slower — but the manufacturer's schedule and warranty terms don't move. Rheem still excludes scale damage from coverage either way. Softer water buys you margin; it doesn't buy you out of the annual flush.
